You can also just link to it if you prefer. In Google Docs, click the Create New menu and select Form. You have several options such as sharing and embedding. If the size is wrong for embedding, edit the height and width sections of the embed code (they’re very easy to spot). You can also set notification rules to send an email to let you know when there’s been something submitted.
